Forms

This section contains forms that graduate students will need to fill out in the following order at various times during the program. Except for the Program Development Form, the Graduate Coordinator must approve and sign all forms. Consult with your advisor if you have any questions about these forms.

Program Development Form (see: Graduate College Forms webpage)
All graduate students should work with their advisor/department to fill out the Program Development Form in the first semester of study. The Program Development Form will serve as a plan of study for the student. A properly completed Program Development Form form includes the exact final list of courses that are to be applied to meet the credit requirements of the graduate degree program including all MSE core courses (MSE 505, MSE 508, MSE 518, PHYS 515, PHYS 523) and organized in the following manner.

1. Core Course (add credit subtotal)
2. Elective courses (add credit subtotal)
3. Thesis credit (only 6 credits)
4. Total number of credits

Note: If the student is substituting a course for one of the core courses, the student and the student’s advisor must submit a Request for Adjustment of Academic Requirements (see: Graduate College Forms webpage) detailing the substitution and submit it with the Application forAdmission to Candidacy form.

Supervisory Committee appointment form (MS students only; see: Graduate College Forms webpage)
All MS graduate students should work with their advisor to fill out the Supervisory Committee appointment form by the time the Application for Admission to Candidacy for Graduate Degree is submitted.

Application for Admission to Candidacy (see: Graduate College Forms webpage)
A student may apply for admission to candidacy after completion of all MSE core courses (MSE 505, MSE 508, MSE 518, PHYS 515, PHYS 523). A properly completed Admission to Candidacy form includes the exact final list of courses that are to be applied to meet the credit requirements of the graduate degree program and organized in the following manner.

1. Core Course (add credit subtotal)
2. Elective courses (add credit subtotal)
3. Thesis credit (only 6 credits)
4. Total number of credits

Example of how to fill out this form.

The form must be submitted no later than the semester prior to the semester of graduation (see exact deadlines in the academic calendar). A student must first be admitted to candidacy before scheduling or participating in a final oral or written examination.

 

Note: Only for older Catalogs - Report of Culminating Activity (see: Graduate College Forms webpage)

The Report of Culminating Activity form should be initiated by the student prior to their culminating activity. The form should then be submitted to the student’s advisor so that the advisor and supervisory committee can note the completion of the culminating activity.
Students should be aware that the deadline for completion of culminating activities is about half-way through the semester in which the student chooses to graduate (see exact deadlines in the academic calendar).
